That is for drilling with a brace and bit by hand. It lets you get more pressure by wrapping the chain around what you are drilling and making more pressure by turning the threaded part.

Herb beat me to it! Most of these were used for drilling with a metal bit into a piece of bar stock or pipe when away from the post drill in the blacksmith shop. If you have ever tried to drill a hole in metal with a brace and bit you would appreciate this tool adding pressure while you were smearing lard on the hole and cranking on the brace.
