I have tried google but can't find much. What orientation should the piston ring be compared to the wavy ring under it? 180?

Thanks for that. Could the ring move around and possible cause the wavy ring to slightly come out as shown in the photo and score the bore?

I suppose it could move, but that doesn't seem to be a problem with most ring sets. At one time, it was common practice to "pin" piston rings to prevent them from shifting around in the grooves. I think that caused more problems than it solved, with pins coming loose and uneven wear. If you can determine the mfr of your ring set, it would be a good idea to cee if any specific information is available relating to installation. Some mfr websites offer a lot of information, some do not.

You will find that the wave spring behind the ring was fitted as a patch up rebuild at one time for a worn bore.

Could rotate, but ring gap being so small when compressed in the bore, it should not be able to get out

Are you re-assembling an engine with used rings?
