Removeable ear on transmission bands
I had something happen that I thought couldn't happen on a properly adjusted band. While driving down a washboard road, the reverse band ear came off rendering reverse useless. The ear was on the passenger side and this is why I think it came loose. I re-installed it on the drivers side along with tightening up the ear. This also puts the ears under the peddle bosses, giving them no room to come off. The rotation, in this installation, should also help keep the the ear engaged . Just sharing.

Detachable band ears should always be on the pedal (driver's) side.

RHD are reversed and give no problems, the band it's self was the problem, the ear wasn't fitting correctly.

I agree, the detachable ear should be on the pedal side, but this doesn't sound like something that could happen with properly adjusted bands in either case.
The band is under tension (trying to open) and held in place with the pedal shaft, spring, washer and nut, all acting to force the detachable ear to stay attached as it holds the band in place preventing it from springing open .
For the ear to become detached, force would have to be exerted on the ear in the opposite direction (forcing the band open instead of holding it closed) allowing the ear to slide off the flanges becoming detached. I can't see how that's possible without a seriously weak spring and/or maladjustment (too loose).
I'd definitely check that spring. I'm certainly no expert, but this doesn't sound right to me.

If you look at a removable band you will see that the end of the band is clipped into the ear which in turn makes it the same as a riveted ear.
